The last shift check

Take the last shift and pick one truck or loader. Answer five questions, and write down the time each answer came in. The one that came in last shows where to start.

How much did it move?
How long did it stand, and why?
What did it take in diesel?
Did it go to the workshop, and what was done?
Who ran it, and for how long?

Five questions follow, each with a line for the answer, a space for the time the answer came in, and the number of the fix it belongs to. How much did it move? How long did it stand, and why? A box at the bottom has room to note who you had to ask and where you looked, and a line for when the last answer came in.
